What is IVF? And How Does it Work?

IVF has traditionally involved retrieving numerous eggs through ovarian stimulation to create multiple embryos. This allows for the transfer or cryopreservation of several embryos, ultimately enabling the selection of the best embryos through blastocyst culture. Ovarian stimulation is used to boost the quantity of eggs in order to address the limitations of the IVF process, allowing the choice of single or multiple embryos for implantation.

The prolonged protocol is always linked with the potential for issues such as ovarian hyperstimulation syndrome (OHSS), which can also result in increased dropout rates, emotional strain, and abdominal discomfort. Some experts believe that there is a higher occurrence of low birth weight and birth abnormalities after undergoing IVF therapy. Natural Cycle Stimulation for IVF

Natural IVF involves monitoring the natural cycle and retrieving one egg before or after LH surge with an antagonist and hCG trigger to avoid OHSS and multiple pregnancies. The cost per cycle is reduced to one-quarter of a stimulated cycle. To enhance success rates, oocytes can be collected in multiple natural cycles, followed by multiple frozen embryo transfers, particularly for elderly poor responders. The main drawback is the early LH surge, which can lead to premature ovulation and reduce the chances of a successful oocyte pick-up. Despite this, in certain situations, the pregnancy and live birth rates can reach up to 45% and 32% through this safe and low-stress stimulation method. The use of antagonists in a natural cycle may be referred to as a modified natural cycle, which benefits patients with previously limited ovarian response to traditional stimulation.

Best IVF and Fertility Center cares about your Economic & Psychological Consideration

Mild stimulation is evidently more cost-effective compared to conventional treatments, with minimal risk of OHSS. Additionally, it reduces the likelihood of multiple pregnancies and preterm birth. Proper counseling can alleviate the stress of cycle cancellation in natural cycles. Mild stimulation also decreases the dropout rate and lowers the risk of depression compared to standard IVF treatment. Patients undergoing mild stimulation handle treatment failure positively and remain hopeful for future attempts due to reduced psychological burden. Such extensive considerations for your physical and financial issues makes NEX IVF one of the best fertility centers in Patna.

When to Refer IUI cases to IVF?

Intrauterine insemination (IUI) and in vitro fertilization (IVF) are the most popular infertility treatments available today. There are no unique solutions that fit every solution to infertility and the path you will take will be unique to specific cases, but there are some common causes.

What is an IUI?

Intrauterine insemination is a method of assisted reproductive technology. It involves placing processed semen into the upper uterine cavity to help sperm reach the egg. IUI is a cost-effective and noninvasive treatment for various fertility issues, with pregnancy rates ranging from 10% to 20% per cycle. However, it may not be suitable for certain cases, such as women over 35, severe male infertility, blocked fallopian tubes, and other specific conditions, in which case in vitro fertilization (IVF) may be more appropriate. Additionally, IUI can be performed with or without ovarian stimulation. Controlled ovarian stimulation combined with IUI can offer better pregnancy outcomes, but it also comes with potential complications such as multiple

pregnancies.

Number of IUI Cycles

The National Institute for Health and Care Excellence (NICE) fertility guidelines 2013 advocate for up to six IUI cycles for patients with unexplained infertility, male subfertility, cervical factor and minimal to mild endometriosis. If no pregnancy is achieved by the end of Fourth to Sixth cycle, patients should be offered IVF. In certain studies, it was shown that Women who were offered IVF after their third IUI cycles, the cycle fecundity increased to 36.6% per cycle compared to 5.6%, if they were offered another three IUI cycles.

NICE GUIDELINES 2013

Women aged under 40 years who have not conceived after 2 years of regular unprotected intercourse or 12 cycles of artificial insemination (where 6 or more are by IUI), offer 3 full cycles of IVF, with or without ICSI. If women reach the age of 40 during treatment, complete the current full cycle but do not offer further full cycles. In women aged 40–42 years who have not conceived after 2 years of regular unprotected intercourse or 12 cycles of artificial insemination (where 6 or more by IUI), offer 1 full cycle of IVF with or without ICSI, provided the following three criteria are fulfilled:

  1. They have never previously had IVF
  2. There is no evidence of low ovarian reserve
  3. There has been a discussion of additional implications of IVF and pregnancy at this age.

Where subsequent study shows that there is no chance of pregnancy with expectant or IUI treatment, and IVF remains the only effective treatment, we refer the Women to IVF.
